Application Forms

Application for New Admissions, Re-Admissions, Transfer, and Concurrent, use APPLY NOW button on homepage. Be prepared to upload all documents at time of submission; application may not be submitted without upload of all required documents (drivers’ license or other photo ID; SSN card or proof of SSN; high school transcript; ACT test scores or other; copy of other college transcripts if appropriate; Concurrent Permissions Form if appropriate; Immunization Form if not high school graduate).

Concurrent Enrollment

High school seniors and juniors may take classes at CASC while in high school when their high school GPA is (minimum) 3.0 or 19 ACT composite or 990 SAT. Homeschooled students are considered a Junior or Senior depending on anticipated secondary school completion. Students should consult with their high school counselor and parent/guardian, complete the application, and return all credential requested on the application. Tuition waivers for SENIORS are awarded on their first 18 cr-hrs taken during the Summer 2022, Fall 2022 & Spring 2023; JUNIORS are awarded a waiver of tuition on their first 12 cr-hrs taken during the Summer 2022, Fall 2022 & Spring 2023 semesters with CASC.

Summer and Fall applicants should complete the Permissions Form and be prepared to upload this document and other documents (ID, SSN, high school transcript, ACT Test scores, and other college transcript if appropriate) to ONLINE APPLICATION. Application cannot be submitted without an upload of all required documents.

Concurrent to First Year Freshman and New Concurrent Students: should use the on-line APPLY NOW application found on the homepage. Applications cannot be submitted without upload of all documents.

Graduation

Students approved for a Fall 2023, Spring 2024, or Summer 2024 graduation may participate in May 11, 2024 Commencement. An approved application for graduation must be on record.

Apply for Graduation

Student may apply for graduation in one of two ways: 1) by active Graduation Application link found in their myCarlAlbert portal account or 2) by paper Graduation Application form (download a copy below).

Online Application:  Students may apply for graduation using the Graduation Application link found in their myCarlAlbert portal account. If this link is active, then the system’s degree audit has determined the student will meet requirements for graduation with their current course schedule.

Paper Application: If the online Graduation Application link is not active to a student who is eligible to graduate, then that student should complete a paper Application for Graduation and submit it to graduation@carlalbert.edu or Office of Admissions. For these students, the Registrar will typically adjust their degree audit for advanced standing coursework, course substitutions, elective credit recognitions, transfer coursework, or other exceptions not recognized by the system’s degree audit.

Once approved, the student should review the Graduation Application portal information and confirm the spelling of their name and address for diploma printing and mailing, and monitor their email for any updates. Registrar will notify student by CASC email concerning graduation/degree pre-approvals and conferral, Commencement, diploma, and any updates.

Commencement

Commencement is schedule for Saturday May 11, 2024, at 11 am and 1 pm.  The ceremonies will be held in the Hamilton Auditorium. Students who are eligible to participate in the Commencement (Fall 2023, Spring 2024, and Summer 2024)  will be emailed (before late March) with instructions on Commencement. Students may purchase cap, gown, honor cords, and other graduation supplies from the Bookstore on the Poteau or Sallisaw campuses. Students should monitor their CASC email and personal email for notices concerning graduation updates and award, when to pick-up/mail diplomas, details on Commencement, etc. So, check your email!

Degrees

Degrees will be conferred and posted to the Spring graduate’s official transcript by Wednesday the week after semester classes end.  Student may order a transcript and choose to have an official transcript sent after grades and degrees are posted.

Diplomas

Diplomas will be printed and mailed to all graduates once they arrive on campus and are available for personal pick up or mailing (typically 4-6 weeks after end of semester).

FERPA Forms

FERPA (The Family Educational Rights and Privacy Act) is a federal law concerning the release and access to a student’s educational records. FERPA protects students’ rights regarding the privacy and accuracy of education records.

Prior Learning Credit

In compliance with State Regent policy, CASC recognizes the need to evaluate learning acquired from outside the traditional classroom, including the following testing or learning evaluations.

To receive credit, students must be enrolled or eligible to enroll, and submit official test results or learning evaluations to the Office of Admissions. Credit is posted to the CASC transcript after completion of 12 credit hours with CASC and only for subjects available for completion at CASC. The transcript will identify credit award and assign appropriate CASC course number as “P” grade. Prior learning credit earned from another institution will be transferred on the same basis to CASC when course exists in CASC College Catalog.
Tax Return & IRS

If you were selected for Verification and are required to submit a student and/or parent IRS Tax Return transcript you can request one online at the link below, complete and mail/fax in the 4506-T Form below, or by calling 1-800-908-9946. Select “Option 2” if requesting by phone.

If you or your parent did not file a tax return, you must submit an IRS Nonfiling Verification Letter by submitting the IRS 4506-T Form below.

– Non-filer: Only required for independent students and parents of dependent students.
– Tax Return Transcript or IRS 1040 Form: required for all students/parents that filed a Tax Return.
